To Register for Benefits

Covenant Cupboard relies on governmental agencies plus other businesses and organizations for the food and support we give your family.  Some of these organizations require us to certify residency and other information, and request periodic reports from us.  Therefore, we require the following when you register, and may periodically request that you furnish updated information.

• Proof of residency, such as a current utility bill or rent receipt received in the mail that shows your name and address.  A driver's license is not acceptable.
• Proof of family size and ages, such as driver's license, birth certificate, or other governmental agency form showing birth dates for you and each member of the family unit.
• Proof of income, such as a payroll check receipt, a letter from the Social Security Administration containing your monthly Social Security payment, or a copy of a court or other legal paper containing income information.

Failure to provide the necessary documentation will result in Covenant Cupboard being forced to terminate your opportunity to receive support.
